Transanal Total Mesorectal Excision (TaTME - Cecil Approach) is a novel operative approach to radical surgery for rectal cancer, and has been a hot topic in colorectal surgery for years. Even though the principles of TME can be followed by using a laparoscopic minimally invasive approach, surgeons encounter difficulties in some cases: obese and male patients, narrow pelvis, and specially for the distal margin section. TaTME (by Cecil Approach) aims to solve some problems with the previous TME, such as exposure of the peripheral mid-lower rectal space, calculation of the distal cutting margin, and carrying out a completely minimally invasive operation, overcoming the lack of visibility in the small pelvis and theoretically improving the rate of radical resections. Cohort studies have shown feasibility concerning procedure, specimen quality and morbidity; in addition to a good quality of life and functional outcomes after surgery.
