Transanal approach for rectal cancer is a hot topic among colorectal surgeons, there are several advantages for going “down-to-up” for the mesorectal dissection in terms of better distal and circumferential margins.
Combined transabdominal and transanal dissection improves maneuvers in cases of narrow pelvis or previous surgery, but to obtain these advantages mentoring, experience and a learning curve is needed.
