An adequate surgical treatment with highest curation rates, is as important as a surgical treatment with the lowest complication rate.
Radical Cystectomy is a surgical technique with a high morbi-mortality rate, but is it only important the technique we perform? Can we set up more items? besides of how we technically perform it with our hands.
As more complex and aggressive the treatment is, we need to prepare the patient, moreover in the oncological patient profile, which the most important risk factor is tobacco consumption (Smoking).
Moreover, in the last years, neo-adjuvant chemotherapy, previous to surgery, has made even more aggressive muscle invasive bladder cancer treatment. That’s one of the reasons why radical cystectomy has to be seen with a different perspective when compared with respect the other oncological surgical treatments, in which surgical technique is the key. In radical cystectomy, we must have a wider view before starting the procedure, and after.
Furthermore, we believe is very important, that not only the surgeon (urologist) is going to be key in the development of the procedure, but also de anesthesiologist (among others), have an important role which might determine some complications, and therefore, the chance to avoid them.
Therefore, radical cystectomy does not start and finish in the surgical technique and the urologist, who is the responsible, but also other collaborators, which makes the purpose of the treatment success a more complex and encouraging, don’t you believe so?
