Gastroesophageal reflux (GERD) is currently regarded as one of the main medical complications after sleeve gastrectomy (SG). Pre- and postoperative diagnostic studies in obese patients to diagnose preexisting and de novo GERD are still a matter of controversy.
Conversion to Roux-en-Y gastric bypass (RYGBP) is considered the gold standard for treating severe GERD despite optimal medical treatment.
In this video we present a conversion of SG to RYGBP using the DaVinci Xi robotic platform for severe GERD in a patient with a BMI of 30 Kg/m2. Preoperative esophago-gastro-duodenoscopy revealed gastric mucosal atrophy, which was confirmed in the pathology report. It was decided to also perform a gastrectomy of the remnant stomach.
