A 48 year old female patient with a history of HBP, DVT, anemia due to iron deficiency and a previous cholecystectomy underwent a Laparoscopic Roux-en-Y Gastric Bypass (BMI 57 Kg/m2) in 2007. The patient lost weight and reached a BMI of 45 Kg/m2. Later on she suffered from weight recidivism, and her current BMI is 51 Kg/m2. Upper endoscopy and a volumetry by CT-Scan revealed that pouch characteristics were normal. The patient does not take the medication recommended by her endocrinologist regularly and her habits are not healthy habits. What is the most reasonable option to follow right now?
