In this SOC, Dr. Balibrea reviews the main features and advantages of three basic elements in the equipment needed to perform revisional bariatric surgery procedures: energy devices, endo-staplers, and barbed sutures.
The video provides an in-depth review of the usefulness of the Harmonic scalpel, mainly for hemostasis and dissection, which is especially difficult in patients with previous surgeries. The Harmonic Ace +7 scalpel® is a device with optimised ergonomics that not only provides maximum versatility for dissection, but also has an advanced hemostasis option that is especially useful in revisional surgery.
He also reviews the advantages of using versatile endo-staplers such as the Echelon Powered® that allow for maneuvers that are sometimes complex, using suitable loads for tissues that often have different characteristics from the usual ones. As an easy-to-use device, it adapts to all situations that may arise during this type of surgery. In addition, the use of GST®-type loads ensures optimal tissue traction, preventing tissue displacement and ensuring that the geometry of the staple line is optimal.
Finally, the video reviews the usefulness of knotless barbed sutures such as Stratafix®. In many revisional procedures, manual suturing is required, and the use of barbed sutures is an optimal solution for suturing and anastomosis, thus ensuring maximum patient safety. Barbed sutures avoid having to pull the suture during suture placement, as well as having to to tie knots.
This is an interesting overview of the instruments required for technically demanding surgeries in which it is imperative to minimize complications.
